Medical Emergencies

Our patient portals are meant for routine health management. You should not use patient portals for anything requiring urgent attention. For medical emergencies, please call 911 immediately. Otherwise, follow up with any questions regarding your health with your primary care provider. If you do not have a primary care provider, please view our list of providers.

These portals are for your inpatient hospital stays only.

St. Luke's Campus

Inpatient Hospital Visit Patient Portal

Mohawk Valley Health System is pleased to offer patients and their designated family members a secure patient portal to view their medical information from their inpatient stay at our St. Luke's Campus. The portal includes:

  • medications and diagnosis at discharge 
  • height, weight, blood pressure, etc. 
  • patient education and discharge instructions 
  • select laboratory results 
  • contact information for next care providers.

The portal includes patient information from Monday, July 21, 2014, to the present date. As the electronic health record functionality expands we plan to add more features to this new tool.


St. Elizabeth Campus

Inpatient Hospital Visit Patient Portal

Mohawk Valley Health System is pleased to offer patients a secure portal to view their medical information from their patient stay at our St. Elizabeth Campus. The portal allows patients and their designated family members to review health information from their stay at SEMC. The portal includes:

  • Information about medications and diagnosis at discharge
  • Patient education and discharge instructions
  • Laboratory and radiology results
  • Contact information for providers who will continue your care after you are discharged.

By using our patient portal, you can review your medical information, including a list of your current medications, allergies, immunizations, your diagnosis at discharge, height, weight, body mass index (BMI), temperature and more. You will also be able to review test results, select laboratory results, and discharge instructions.

Patient Portal Registration

Follow these steps:

  1. Type www.app.relayhealth.com into your browser to get to My Patient Portal
  2. Choose Register Here at the bottom of the sign in screen.
  3. Click on the Patient icon.
  4. Fill out all required fields and choose security questions. Complete your personal information and click Register.
  5. Once you register, choose the Providers icon and click Add Provider.  Fill in the two fields: St . Elizabeth and the zip code 13501. Click Search.
  6. Click Select to register as a patient at SEMC.
  7. A request will be sent back to St Elizabeth to accept you into the practice. Once this is done, you'll get a confirmation email.
  8. The link enclosed in the email will take you to the Patient Portal home page. Log in with credentials you created during registration to complete your health record.
  9. Congratulations! Your account has now been activated.
