In the Town of Webb, health resources are scarce. Mohawk Valley Health System (MVHS) operates the only primary care office in Webb, and travel time to specialty services, urgent care and emergency care outside the region can be excessive.
MVHS is responding with an innovative, new initiative that will provide a comprehensive continuum of care to the Town of Webb. The program will bring together regional healthcare partners to combine service efforts and skills, bridge service gaps and coordinate care for high need persons in the community. MVHS joins six regions of the state in piloting this Collaborative Model of Community Medicine and Paramedicine, being generously funded through a special grant by the Mother Cabrini Health Foundation, and assisted by the Iroquois Healthcare Association, the Home Care Association of NYS and the IPRO health care improvement organization.
Through MVHS’s Rural HealthCare Partners Program, MVHS is partnering with Old Forge Volunteer Ambulance Corps, the MVHS Town of Webb Medical Office and Central Adirondack Partnership for the 21st Century, Inc. (LivingADK) to create a foundation of a mobile integrated healthcare (MIH) team, which will expand healthcare coordination in the Town of Webb to bring care to people in their homes who may not be able to access care any other way. The partnership brings together EMTs/paramedics, home healthcare services, discharge planners, primary care and preventive healthcare services.
“When healthcare agencies and organizations work together to deliver healthcare in a collaborative environment, we can help improve the quality of medical care in a region like the Town of Webb, where resources are scarce,” said Lisa Volo, senior director of MVHS Behavioral Health and Community Services.
The goal of the MIH initiative is to provide the appropriate care and referral services needed in the Town of Webb in order to reduce the number of unnecessary emergency transports and unnecessary hospital admissions and/or readmissions.

The Wynn Hospital will strengthen and grow full service cardiac offerings, support the development of a Neurosciences Institute that will include a Neurovascular Center and Comprehensive Stroke Center, will be a world class diagnostic center, including Interventional Radiology and Advanced Endoscopy and provides a Level III Trauma Center plus the ability to serve 90,000 emergency visits per year.
